The Questions Everyone Has Before Their First Post-Lipo Massage
If you've recently had liposuction — or you're planning to — you've probably heard that post-surgical massage is important. But if you're like most of my clients, you're showing up to that first appointment with a head full of questions and maybe a little anxiety about what you're walking into.
Is it going to hurt? Do I have to take my clothes off? Can I even come in if I still have drains? These are exactly the right questions, and I want to answer all of them — because an informed client is an empowered one, and getting the best possible outcome from your surgery starts with understanding your recovery.
Here's what to expect before, during, and after your first lipo massage at Healwell Portland.
Will It Hurt?
This is the question I hear most often, and the answer genuinely surprises people: no. Lipo massage is not painful.
Manual lymphatic drainage is a light, gentle, skin-stretching and stroking technique. We are working with the lymphatic system, which sits just beneath the surface of the skin. The pressure is precise, but it is never deep or forceful. If you were imagining something that feels like a deep tissue massage applied to a tender, post-surgical body, let that image go entirely. This is the opposite of that.
In fact, most of my clients find their first session deeply soothing — so much so that many fall asleep on the table. The session opens up the major lymphatic nodes and then moves through the body with very specific, intentional strokes designed to encourage fluid movement and reduce swelling. It feels gentle because it is gentle. That gentleness is precisely what makes it effective.
Do I Have to Undress?
You will disrobe and remove any compression you might be wearing for your session, but you will be draped and covered at all times. Only the specific area being worked on will be exposed at any given moment. Your privacy and comfort are protected throughout the entire appointment. Many clients are relieved to hear this — especially those who are still feeling self-conscious about their body in the early days of recovery.
When Should You Start Lipo Massage?
Timing is one of the most important factors in your recovery, and my strong recommendation is always to follow your surgeon's guidance on when to begin manual lymphatic drainage after your procedure. Every surgery is different, every body is different, and your physician's directions take priority over any general timeline.
That said, most of my clients begin within the first five days post-surgery. A typical treatment schedule looks like this:
Two sessions per week for the first two weeks
Weekly sessions continuing after that
Six to eight sessions total is common for most patients
Starting MLD early has real, documented benefits. Research has shown that beginning lymphatic drainage soon after surgery can help patients have their drains removed sooner — a meaningful improvement in comfort and quality of life during those early recovery days. Early MLD also plays a significant role in preventing fibrosis, the hardening and thickening of tissue beneath the skin that can lead to lumps, bumps, and irregular texture that is very difficult to correct after the fact.
Can I Come In If I Still Have Drains?
This is a question I get often, and the short answer is: sometimes, yes. We can work with clients who still have surgical drains in many cases. But as with everything related to your recovery timeline, your surgeon has the final word on what is appropriate for your specific situation. Do You Need Your Surgeon's Permission?
I always recommend getting your surgeon's approval before beginning post-surgical massage, and at Healwell we defer to your physician's directions on timing and any restrictions. This isn't just a formality — it protects you and ensures that your massage care is coordinated with the rest of your recovery plan. Most surgeons are supportive of MLD and many actively recommend it. If yours hasn't mentioned it, it's worth bringing up at your next follow-up appointment.
What Happens If You Skip the Aftercare?
I want to share something I've seen in my practice, because I think it illustrates better than anything else why post-surgical care matters so much.
I've worked with clients who came to me a year or more after their procedure — not for routine aftercare, but because they were dealing with the long-term consequences of having none. One client had liposuction on their arms. Without post-operative lymphatic care, they developed significant fibrosis and cording, a specific type of scar tissue that forms in bands or cord-like structures beneath the skin. They were visibly uncomfortable in short sleeves and deeply self-conscious about the results. They had invested so much in their surgery — financially and emotionally — and the outcome they hoped for was compromised because the aftercare simply wasn't there.
We can do scar work after the fact, and it can reduce the appearance of fibrosis and cording. But the most effective time to prevent these outcomes is immediately after surgery, not months or years later. Clients who begin MLD early and stay consistent with their sessions have smoother, more even results. The investment in your recovery protects the investment you made in your surgery.
What to Expect During the Session Itself
How the massage is performed
At Healwell, your session begins with opening the major lymphatic nodes — typically in the neck, underarms, and groin — to create pathways for fluid to drain. From there, we work through the treated areas using light, rhythmic strokes that follow the direction of lymphatic flow. The sequence is deliberate and precise. Nothing about it feels random or rushed.
What you will feel
Most clients describe the sensation as deeply relaxing, almost meditative. The pressure is light and the movements are slow. You may notice some mild achiness or tenderness in areas that are still swollen or healing, but the massage itself should not cause pain. If anything feels uncomfortable, always let me know. We can adjust.
How long it takes
A typical session runs about sixty minutes, depending on the areas treated and where you are in your recovery. Your first appointment will also include time for intake and a brief assessment so I can understand your surgery, your timeline, and any specific concerns you want to address.
